If you are a part of a non-profit,
community organisation, group or service on the Sunshine Coast and would
like to add details to

details will be sent to the CIS Coordinator and added to the CIS
database. Alternatively you can print the PDF Form, fill
out the details, sign and post or fax to CIS (address on form).

Please note: it is not necessary to complete entire form, only those
details that have changed along with your organisation name.
